Since series one, Alan has lost over 500lbs and is back for a double mastectomy in his quest to walk for the first time in years. 22 year old Vanessa is hoping to revive her sex life by removing her sagging skin, whilst Kim a school principal is getting surgery to remove her "bat wings".
Tiffany, who weighs 650lbs and featured in the first series returns as the doctors struggle to convince her to enter an extreme weight loss facility before it's too late. Kathy tries to rid herself of the excess skin caused by massive weight gain after her daughters’ suicide, and Marshall, one of the doctors' biggest patients from series one, visits the centre 11 months after his surgery to show just how much progress can be made in a year.
